Part One: Online Enrollment

As part of the online enrollment process, you will review and update your household profile, medical information, and school policies and agreements. Please pay particular attention to the following:

  • Address and Contact Information for the student and parents
  • Changes such as divorce or remarriage - note via the "My Family Has Changed" section
  • Medical Information
  • Emergency Contacts - 3 are required!
  • Permissions and Agreements
  • Transportation Arrangements including bus deposits and student driver fees

Part Two: Proof of Identity (New Students only)


Prior to starting school at Wakefield, you must present Proof of Identity to a Wakefield staff member in person. They will make a copy for your student's file and immediately return the original to you. Proof of Identity may be an original birth certificate or a valid passport. If you are new to Wakefield, please present proof of identity as soon as you are able, or if you are moving to the area from out of state, as soon as you are able to visit campus.

Part Three: Health Form Requirements

Commonwealth of Virginia School Health Entrance Form and Proof of Immunizations (click to download): The Commonwealth of Virginia School Entrance Health Form must be completed in its entirety for:

  • students who have moved to Virginia from out of state
  • students entering Junior Kindergarten, Kindergarten, 6th grade, or 9th grade.
  • students with ongoing medical issues
  • students whose last form was based on a physical performed more than 2 years ago.

  • The student's comprehensive physical must have been completed within the past 12 months. Please complete the form in its entirety. Incomplete forms will not be accepted. Students will not start classes until records are complete and approved by Student Health Office. Medical exemptions require additional paperwork, please contact Angela Gerald at agerald@wakefieldschool.org for details.

    Students who enroll after 20th August or midyear will be granted a 15-day grace period for submission of the School Entrance Health Form as long as parents have made every effort to secure a doctor’s appointment before the first day of school. However, current and up-to-date immunizations, emergency information, and the Student Health Update must be submitted before students may attend school. Special exemptions and/or arrangements are handled on a case by case basis with the Health Office designee.

    Sports Physical (click to download)

    The sports physical must be dated May 1, 2017 to present, and is required (as listed below) in addition to the Commonwealth of Virginia School Health form. Although the form refers to the student as being in High School, it is also the form to be used for 6th to 8th grade students.

    Who must submit:

    • Students entering 6th grade through 9th grade
    • Students enrolling in the Fitness for Life or Advanced Physical Education courses
    • Students playing Varsity or Junior Varsity Sports

    Allergy Emergency Care Plan (click to download)

    If applicable, the Allergy Emergency Care Plan must be filled out and signed by a physician. Students will not be allowed to carry Epi-Pens, Inhalers, etc. without the proper paper work.

    Student Asthma Card (click to download)

    If applicable, the Student Asthma Card must be filled out and signed by a physician. Students will not be allowed to carry Epi-Pens, Inhalers, etc. without the proper paper work. Inhalers need to have a pharmacy label directly on the inhaler itself.

    Religious Exemption from Immunizations (click to download)

    If the administration of immunizing agents conflicts with your family’s religious tenets or practices, please contact Angela Gerald for information on submitting this form.
